Office Policies

Our office is conveniently located at 142 Beaver Creek Place, Suite 112 at Doctors on Call. Our friendly and efficient Front Office Staff will assist you to arrange an appointment that is most convenient. To schedule an appointment please call us at 970-949-5434.  Please bring your insurance cards and medications to your appointment.

Appointments & Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment I request the courtesy of 24 hours notice. In such a case, please call my office and reschedule your appointment with one of my patient care coordinators.

Emergencies

My office provides on-call  phone contact for emergencies. Call 970-949-5434 so that the physician on call can be contacted.

Accepted Insurance Plans

My practice accepts most major insurance plans and most of the smaller insurance plans. If I do not participate with your insurance and your plan allows out of network benefits, my office staff will assist you with the necessary paperwork so you can receive reimbursement directly from your insurance company.

If you do not see your insurance listed below, please call the office to speak with one of my knowledgeable staff members about your individual coverage.
